My Black Thursday
An overwhelming day...
Pt 1:
- Nepali (doesn't really speak malay/ english)
- came in complaining of pain on LL8, saw GP 3 days ago, taking antibiotics and painkillers
- pain on eating, but no pain on visualising food
- Cinically: No caries, no TTP
- Periapical Radiograph: No periapical lesion & caries on LL5,6,7,8
- Extraorally: swelling on L cervical resion, fixed, hard
* ??? Submandibular gland obstruction?? Lymphadenopathy?? Difficult diagnosis!!!
Luckily it was Dr. Araq (our mighty oral surgeon)'s day of visit...
Straightaway REFER!!
Differential Diagnosis: ?? Pharyngeal Lymphadinitis? arising from a lymhatic granuloma? (arrhhh i cant even hear the medical term clearly as our Dr Araq was so soft spoken) -- but he is a very nice Oral Surgeon tho.. pheeeew
Pt 2: Xla LL7
Have never seen a root that curved in my whole practicing life (not that i've started working for long) but still...
It's properly gripping to the alveolar bone
Difficult extraction >.< - For Sure!
called for SOS..

Pt 3:
* before seeing the patient, my assistant kindly warned me about the patient having psychiatric problem. In my head i was thinking: Schitzo? Learning difficulties? hm.. no doesnt seems like it
So i went on finding out more about him
- Pain on UL quadrant, below orbital bone, around zygomatic area
- No Dental problems
- Came in requesting for a simple filling under LA
- MH: Nerve damage due to an accident few years ago, had surgeries, on all sorts of pain killers i.e Gabapentin, but no help, seeing neurosurgeon soon
Tx provided: Simple composite filling to restore aestetic.
So he's not psychic..
However pt keep requesting for LA!!!!!
After talking to him for 15 minutes explaining that we're not in the position to give him an LA for 'no reason' as there is no indications for LA. He left.
After finishing my one last patient. I discussed the case with my seniors.
Just as soon as we stopped the topic.. The patient turned out with his dad (claiming that he's who who who from which which residential area) grrrr
Luckily my senior was there..
Again he was asking if we can give him any form of aneasthetic, just to relieve the pain.. or else if he can get hold of a load of aneasthetics and inject himself
We actually felt bad for him..
He was that desperate....
Another 20 minutes spent explaining to him --- Best thing to push it all to the SPECIALIST!!
sigh.......
Dr Lim immediately reach out for her ice-cream in her storage after they left - Destress
So my way of destress?
Blasting my music in the car! yeaaaah!
At the end of the day, i manage to pull a smile seeing 3 monkeys walking on the electrical cable beside the street on my way back.
so cute and entertaining! =)

Nah... i'm happy that i'm still alive to see the beauty of mother nature. ^^

new record!
just realised i've treated 35 patients yesterday! yay!! new record!
getting faster =)
was posted to relief a small Felda village, Waha this morning.
purely extractions and examiations..
Knackered..
It was a good experience treating in a village as i love travelling.
More to explore.
Was pestering our driver to find us some sweet potato as Waha is known for it.
On our way back, Jai and Azman were busy looking out for a stall.
My lovely dental receptionist bought me some =D
Happy.. ^^

03/05/11 stressful day, happy day ^^
one of the most stressful day at work
a long weekend with a Monday holiday turned our dental clinic into a war zone
more than a hundred outpatents to be seen by 2 dentists and plus another one helping out when she is free..
madness...
seems like i have been having difficult extractions for the whole day - an UL4 with long curved roots, a carious partially erupted LL8, a vertically fractured UR7 which snapped when i was extracting with forceps..
grrrrrr.. roots!!
Another senior saved my broken roots
A clever idea of splitting the tooth into half saved my trouble taking an 8 out!
phew... i did learnt
we ran late... again... leaving the clinic at 17:07 is late! (this is the M'sian working culture in the government)
adrenaline rush for 10 hours
thinking about tomorrow's speech
fighting with non-working scalers
pet talking a crying kid who doesn't even want to open her mouth
drilling and filling teeth with massive holes

i'm drained.
however it was one of the best day i had.
My favourite tunes playing at the background kept me going..
A braved little boy behaved real well when i was doing an extraction for him. =) He was happy when i gave him his tooth as a souvenier.. lol
Was talking to my assistant about tomorrow's speech before leaving a clinic. A Paediatric dental nurse overheard our conversation and told us that the speech was cancelled!
Thrilled, I gave my assistant a big hug!
Off to home... Yaaaaaaaaay!!!!!
